Application Forms

Below are the template forms for the interconnection process. These represent all the forms that will be required from you by your utility. Individual utilities may use additional forms for notification of completed applications or requests for additional information.

Please note that these forms are for reference only. The forms that you use to apply for interconnection must be obtained by contacting your utility.

Simplified or Simplified Spot Network Process

You must fill out and submit this form to initiate the simplified interconnection review process with your utility. All information must be filled out completely and accurately to initiate a utility review. The review process begins once the utility notifies you of receipt of a complete application.

In lieu of an interconnection agreement, the utility will sign the bottom of your submitted form and return a copy to you. This signifies approval to proceed with installation. A wiring inspection is still required, and a witness test may still be required after the generator is installed before final approval is provided by the utility.

This form must be used to confirm completion of the wiring inspection required after you have received your signed form from the utility.

Expedited Process

You must fill out and submit this form to initiate an expedited or standard interconnection review process with your utility. All information must be filled out completely and accurately to initiate a utility review. The review process begins once the utility notifies you of receipt of a complete application.

This agreement authorizes the utility to proceed with a supplemental review at the costs projected by the utility. This agreement is required only if you are pursuing the expedited review process and your project did not pass any of the supplemental review screens in this process. If all screens are passed, your project proceeds with the system modifications check.

This agreement is signed by you and the utility after your project is approved. The interconnection agreement may vary slightly by utility, but is the same whether you complete an expedited or standard review.

Standard Process

This agreement authorizes the utility to proceed with an impact study under the standard review process. If you elect to pursue a standard review or are moved from the simplified or expedited review process to the standard review, an impact study will be required.

This agreement authorizes the utility to proceed with a detailed study if required following the impact study.
